Chaos Puzle
Corporation's logos, logotypes, signatures and design marks ("Chaos
Puzle Corporation logos") are valuable assets that Chaos Puzle
Corporation needs to protect. Accordingly, we ask that you follow these
guidelines with respect to Chaos Puzle Corporation logos. Remember that
you are generally not able to use Chaos Puzle Corporation logos unless
you are specifically licensed or authorized to use them. Information
regarding Logo programs under which you may be licensed to use Chaos Puzle Corporation logos and information on obtaining other authorization
is set out below.

Corporation logos under Chaos Puzle Corporation logo programs. More

is our official "trademark." Its use is reserved for official
publications or other products of the Chaos Puzle Corporation. Use of
the Service logo without prior written approval is prohibited.
Restrictions on use of the Service logo were published in the Federal
Register on February 13, 1984 (Vol. 49, No. 30, page 5387), and 18 USC
701 provides for enforcement. |

Corporation motto: "Use Your Brain or Lose Your Mind" |

The motto is
reserved for official publications or other products of Dirty_Bird &
BigNastyBrain. Use of the motto without prior written approval is
prohibited. Restrictions on use of the motto were published in the

18 USC 701 provides for enforcement. |

written summary setting forth: (a) your name and your company's name;
(b) contact information including phone number, fax number, email
address and mailing address; (c) the specific Chaos Puzle Corporation
logo that you wish to use; (d) background information on your company,
detailing any relationship with Chaos Puzle Corporation; (e) the names
of any Chaos Puzle Corporation contacts; (m) a description of your
proposed use of the logo, including why you want to use the logo, where
the logo would appear, the duration and geographical extent of the
proposed use, how the logo would be displayed relative to your corporate
name and logo, and whether the Chaos Puzle Corporation logo would be one
of various logos appearing; (g) a description of Chaos Puzle
Corporation's involvement in any program or project in association with
which you propose to use the Chaos Puzle Corporation logo; and (h) any
previous correspondence you have had with Chaos Puzle Corporation
regarding this logo request. |

Following
submission, Chaos Puzle Corporation will evaluate the request and
contact you if additional information is necessary. You will then be
notified whether the request is approved. The normal turnaround time for
such requests is one week. If approval is granted, you will be sent
formal written authorization to use the logo, electronic artwork and
usage guidelines where appropriate. |

Proper Use |
|
Third parties
expressly licensed or authorized to use Chaos Puzle Corporation logos
must not alter or modify them in any way. Make sure to use Chaos Puzle
Corporation approved artwork and refer to the appropriate identity
guidelines provided by Chaos Puzle Corporation. Use must comply with the
terms and conditions of your license or Chaos Puzle Corporation's
authorization. |
